ADAMS, Mrs Janet Elspeth (Jan)
For services to education
Mrs Jan Adams has been working for more than 40 years in education, first as a teacher and Associate Principal, as a public servant in the former Department of Education and Education Review Office (ERO) and in the offices of two education Ministers.
Mrs Adams began working with ERO in 1989, and took several roles, including Acting Manager of the Northern Office, before moving to a management position in the national office. She is highly regarded across the ERO network for her deep pedagogical and evaluative knowledge, her in depth understanding of how the sector operates, and her understanding of what works best for children in the classroom.

AFEAKI-MAFILE'O, Ms Emeline Lilian
For services to the Pacific community
Ms Emeline Afeaki-Mafile’o is the founder of Affirming Works, a not-for-profit organisation which provides Pacifica mentoring services to young people in Auckland.
Since 2001, Affirming Works has supported more than 5,000 youth and children and provided training and employment to more than 100 youth workers. Ms Afeaki-Mafile’o established the Fofola Consultancy which produces independent advice on issues affecting Pacific people, including the Tongan Conceptual Framework for Family Violence in New Zealand. In 2010 she established the Community Café to assist with the fundraising needs of Affirming Works and promote Pasifica innovation and sustainability. The café has since become a centre for the wider South Auckland community. Ms Afeaki-Mafile’o has served as Community Advisor to the Ministry of Pacific Island Affairs and as a member of the Pacific Advisory Group to the Ministry of Social Development.

ALLISON, Mr William Paul (Paul)
For services to sport
Mr Paul Allison has served in a number of high profile sport-related roles, including as the Chief Executive of Sport Otago more than 13 years, during which time he grew the organisation significantly to become recognised as one of the leading regional sports trusts in New Zealand.
Mr Allison was a member of the Sport New Zealand Board from 2007 until 2015. He has been a member of the New Zealand Masters Games Executive Committee, a Trustee of the Winter Games New Zealand Trust and the Halberg Disability Sport Foundation, and a steering committee member of the New Zealand Physical Activity Ministerial Taskforce. He has also served more than 20 years in radio and television broadcasting as a radio commentator at Carisbrook, and continues to provide rugby commentary for the Radio Network in 2015. He is a seven-time winner of the Dunedin Harbour Marathon, and represented New Zealand as a marathon runner internationally when he won the Pacific Rim marathon title in Long Beach, California in 1993, and when he came 23rd in a field of more than 27,000 runners in the New York Marathon.

BLOGG, Mr Kevin John
For services to people with disabilities
Mr Kevin Blogg has spent the past 20 years assisting people with disabilities in the Canterbury region to find paid employment.
In 2003 Mr Blogg founded the Catapult Employment Services Trust, a supported employment agency working with all disability groups. Catapult now has ten staff and contracts to work with up to 200 clients per annum. He is retired from the Trust but continues his involvement as Patron. From 1996 to 1999 he was the Work Programmes Development Manager at Comcare Charitable Trust. As the Manager at JobConnect (Comcare Trust) from 1999 to 2002 he developed and established a service for people with mental illnesses, to provide support towards finding and maintaining paid employment. This service now has Ministry of Social Development contracts. Since 2004 he has managed the Blogg Charitable Trust with the responsibility to assess funding applications and distribute funds to disability and children’s charities in Christchurch. He was a founding Trustee in 2010 of Reunite Canterbury, which was set up to help support the reunification of refugee families, serving until 2012. In addition to the services he has established, Mr Blogg spends many hours supporting and mentoring individuals in his own time.

BLUNDELL, Mr Phillip James
For services to people with disabilities and the community
Mr Phillip Blundell was left a paraplegic in 1983 following a helicopter accident while serving in the New Zealand Army and has since contributed his time to disability support organisations and the community.
Mr Blundell became involved with the Disabled Persons Assembly (DPA) in 1985 and joined the National Board as Vice Chair from 1986 to 1988. He was the DPA representative during the commercialisation of the New Zealand Disability Resource Centre, and representative to the New Zealand Transport Complaints Board for two years during the establishment of the Mobility Parking and Total Mobility Taxi Scheme. He joined the Wellington and Hutt Valley branch of the New Zealand Crippled Children’s Society in 1988, later becoming Vice President of the National Board from 1994 to 1996. He continues to serve as a member of the Wellington Branch Executive Committee and took up an interim National Board position in April 2015. He was involved with the Heretaunga Rotary Club for 18 years, is a member of the Better Hutt Valley Group and Secretary of the New Zealand Poppy Places Trust. Mr Blundell was an NZCCS Disability Action appointment to the Board of Access Ability from 2003 to 2012 and was Chair for three years.

BLY, Mr William Morgan, JP
For services to the community
Mr William Bly served as an elected representative on local bodies in Woodville, Manawatu and Tararua between 1983 and 1998, including as a Councillor and Chairman for the Woodville County and District Councils, the Manawatu/Whanganui Regional Council, and as Mayor of the Tararua District Council from 1995 to 1998.
Mr Bly was a member of the Woodville Centennial Committee, and helped produce a commemorative Centennial book of the 1975 celebration. He has been a member of the Woodville Domain Board since 1975. He joined the Executive Committee of the Manawatu Centre of the Cancer Society of New Zealand in 1999. He observed a Relay for Life event in Melbourne in 1999 and helped introduce the event to New Zealand, going on to Chair the Manawatu Centre Relay for Life Committee from its inception in 2001. He has served as President of the Centre since 2005 and is the Manawatu Centre delegate to the Central Districts Divisional Executive Committee. He is one of two Trustees on the Ozanam House Trust Board and was Chairman of the Board from 2007 to 2010. Mr Bly was a Land Search and Rescue advisor for the Bush District, and a member of the Dannevirke Power Board Trust and the Meat and Research Development Council.

BOWDEN, Mrs Hayley Rose
For services to football
Mrs Hayley Bowden played with New Zealand’s national women’s football team the Football Ferns from 2003 until 2015.
Mrs Bowden is the fifth most capped Football Fern of all time with 92 caps, was the youngest ever Captain at age 21, and ended her career with 10 goals for New Zealand. She has represented New Zealand at the FIFA Women’s World Cups in 2007 and 2011 and appeared at both the 2008 and 2012 Summer Olympic games, the latter campaign seeing the Football Ferns reach the quarter-finals. She was a member of the team that reached the final and placed second at the Cyprus Cup in 2010, as well as the third placing team at the 2013 Cyprus Cup. She played with the Lincoln Ladies in the Football Association Women’s Super League from 2013 before a shift to Japan in 2015 and a contract with Iwata Bonita. She has played previously with Chelsea Ladies Football Club in England and Ottawa Fury in Canada. Within New Zealand Mrs Bowden has played with Fencibles United and Lynn-Avon United clubs.

BROWN, Mrs Jodi Anne
For services to netball
Mrs Jodi Brown is a New Zealand netball player who retired from international netball in 2015, having played 61 tests in the positions of Goal Shooter and Goal Attack for the Silver Ferns since her debut against Niue in Kingston, Jamaica in 2003.
Mrs Brown was part of New Zealand’s 2003 World Cup-winning team, and won silver medals at the 2007 and 2015 World Cups, and the 2014 Commonwealth Games. She has played elite domestic netball in New Zealand for 13 years and will continue to play in the ANZ Championship with the Central Pulse in 2016. In the National Bank Cup she has played with the Capital Shakers, the Canterbury Flames, the Auckland Diamonds, and then captained the Otago Rebels in 2007. She played with the Canterbury Tactix for the inaugural ANZ Championship season in 2008 and switched to the Waikato Bay of Plenty Magic for the 2010 season. Mrs Brown captained the Southern Steel for the 2012, 2013 and 2014 ANZ Championship seasons.

CAMPBELL, Mr James Clarence (Jim)
For services to conservation
Mr Jim Campbell has contributed services to conservation for more than 50 years.
Mr Campbell has had a long involvement in habitat restoration and water fowl captive breeding and recovery programmes, including Whio (Blue Duck), Pateke (Brown Teal), Grey Teal and the Mute White Swans. He has created many wetlands on his property, the largest being a 20 acre area covenanted with the QEII National Trust, of which he is a Life Member. He joined Ducks Unlimited in 1976 sserving as President, Board Chairman and is currently Co-Patron. This involves assisting with fundraising efforts for Wetland Care New Zealand, helping to create, maintain and restore significant wetlands in Otago (Sinclair Wetland), the Wairarapa, Wellington and Manawatu regions. He has assisted in building strong bonds between conservation organisations in New Zealand, the United States, Canada, Australia and the United Kingdom. He has been a Trustee of the Masterton Henley Lake Trust since 2003 and has given more than 60 years of support to the New Zealand Forest and Bird Society. As Trustee of the Mount Bruce National Wildlife Centre for 25 years Mr Campbell was instrumental in promoting the building of the classroom facility, and the Sir Edmund Hillary Memorial Lectures.

CAMPBELL, Mr Norman Donald (Don)
For services to tertiary education
Mr Don Campbell has contributed more than 30 years to education.
Mr Campbell has served as Chief Executive in two well-known New Zealand polytechnics, most recently at Whitireia Community Polytechnic, a highly respected industry leader on measures of diversity, quality, educational and financial performance and in international education. He also served as Chief Executive at Tai Poutini Polytechnic on the West Coast, where he oversaw a period of significant growth and innovation, and prior to that as Director of Corporate Services at Northtec. Through his work in tertiary education he has supported national and regional development, more specifically, the communities of Porirua, Kapiti and Wellington, the West Coast and Northland by consulting with local industries to provide specialist courses for students relevant to local businesses. He has become a recognised leader in the tertiary education sector through his board roles on the Institutes of Technology and Polytechnics of New Zealand, the Institutes of Technology and Polytechnics Quality, Polytechnics International New Zealand Limited and as an advisor to Education New Zealand. Mr Campbell is also Fellow of the New Zealand Institute of Management.

CHADWICK, Mr John Te Manihera
For services to Māori and the law
Mr John Chadwick was the managing partner of three all-Māori law firms in Rotorua from 1977 as Hingston & Chadwick, Chadwick Bidois in 1985 and Chadwick Law in 2002.
Mr Chadwick now practices law as a Barrister. In 1987 he was the founding President of the Māori Law Society, served on its executive for many years and is its only Life Member. In 1987 he founded Te Waiariki Purea Trust to manage ‘Movin On’, a Hillary Commission pilot programme providing recreational services to young people in Rotorua. He remains Chairman of the Trust after 28 years. He was the founding Chairman in 1987 of the second Kohanga Reo language nest in New Zealand in Rotorua. In 2013 he was a founding trustee of Te Arawa Whanau Ora Trust. He is currently Chairman of the Rotorua Community Law Centre, having served on its Board for 16 years. In 2011 he was a founding trustee of the Community Law Centre national body. Mr Chadwick has served on a wide range of committees with the New Zealand Law Society, advised Government on ‘Te Ture Whenua Māori Act’ in 1993, established the Māori seat on the Family Law Section of the New Zealand Law Society and is a mandated negotiator for Ngati Tama in its Waitangi Treaty Claim.

CHING, Mr Larry Raynor
For services to education
Mr Larry Ching has worked in education for more than 44 years as a teacher and principal and has continued to work as an educational consultant since his retirement.
Mr Ching is a mentor in the First-Time Principals programme, has been a leader of his regional principals group and built a strong professional learning network of secondary principals across the top of the South Island. He was a driving force behind the establishment of the ‘Top of the South Trades Academy’, which provides opportunities for students to combine their academic studies with practical vocational training and has been the local champion of the ‘Education for Enterprise’ teacher-led organisation that provides opportunities for students to apply their skills, knowledge and learning in real life situations. He has been an advocate for and contributor to wider educational opportunities within his community and supported the local Adult and Community Education programme after its funding ended. Mr Ching is an avid supporter of the Rotoiti Lodge Outdoor Education Centre, where he served as Chairperson of the Trust and has been instrumental in ensuring its survival.

CRON, Mr Michael (Mike)
For services to rugby
Mr Mike Cron has been coaching rugby since 1982 and is recognised as one of the most innovative and respected front row coaches in the world.
Mr Cron has been a New-Zealand rugby resource coach since 2004 working with the All Blacks as well as New Zealand’s other national teams, Super Rugby sides, Provincial and Academy teams, and numerous amateur clubs and schoolboy teams. He has been the All Black scrum coach since 2004, and the All Blacks forward coach since 2012, involved with the back-to-back victories at the 2011 and 2015 Rugby World Cups. Mr Cron has been involved with the International Rugby Board to make scrums safer at all levels of the game.

DANIEL, Mr Graeme Herbert
For services to special education
Mr Graeme Daniel has dedicated more than 30 years to the service of special education.
Mr Daniel is currently the principal of Allenvale School in Christchurch – a provider of special education. He led Allenvale through a period of uncertainty to become a leading provider of special education services. He was up until the end of 2015 the President of the Special Education Principals’ Association of New Zealand and is well known across the education sector as a champion for special education, as a thought leader in national education issues and someone who has dedicated himself to giving students with special education needs and disabilities a voice. He was a member of the Christchurch Education Advisory Board and influenced the Special Education Network Review that considered options for the Christchurch special schools. He was invited by the Minister of Education to join the National Cross Sector Forum on Raising Student Achievement. He chaired the Special Education Transport Assistance Reference Group for the Associate Minister of Education. Mr Daniel is a trustee of charitable organisations that support children and young people with special education needs and disabilities.

DIPROSE, Mr Keith Oliver (Kelly)
For services to athletics
Mr Kelly Diprose has contributed to athletics in the Waikato/Bay of Plenty area since the mid-1980s and is a Life Member of Te Aroha Athletic Club and Athletics Waikato/Bay of Plenty.
Mr Diprose was a founding member in 2000 and is a current Trustee of the Waikato Bay of Plenty Athletics Children’s Supporters Trust. He was a Committee Member of Waikato Bay of Plenty Athletics Association and served as Chairman of the Management Committee from 2003 to 2008. He is the Manager of the Waikato Bay of Plenty Grade 13 Inter-Provincial Athletics Team. He was Chairman of Athletics New Zealand from 2000 to 2001. He managed the New Zealand Pacific School Games Team at competitions in Darwin in 1992, Perth in 1996 and Sydney in 1998. He was Chairman of the Organising Committee and Meeting Manager of the North Island Colgate Games and Chairman of the New Zealand Secondary School Cross Country Championships in 1999. He has coached many track and field athletes who have won national titles and international championships in Australia and England in their grades. Mr Diprose has also coached athletes who have represented New Zealand at the International Association of Athletics Federations World Youth and Junior Championships and the 2014 Glasgow Commonwealth Games.

EDMUNDSON, Mr Neil Donald
For services to hockey
Mr Neil Edmundson has been involved with hockey as a player, administrator and coach since the late 1960s.
Mr Edmundson was instrumental in the introduction of one of New Zealand’s first artificial hockey turfs, at Porritt Park in Christchurch in 1984. In 1990 he established the Hawke’s Bay Hockey Artificial Surface Trust to raise funds for, and then administer, an artificial turf. Under his leadership two international standard artificial turfs have been built at Park Island in Napier. The facility has been the venue for test matches and the New Zealand Masters Tournament in 2015. As a player he has represented New Zealand Universities, Canterbury and Hawke’s Bay. He played New Zealand Masters Hockey and was a selector between 1997 and 2013 in age groups from 40 to 55 years. He captained a New Zealand Masters team that achieved a World Championship runner up result. He was Manager of the New Zealand Mens Black Sticks Hockey Team from 1990 until 1998. Mr Edmundson initiated the establishment of the Kelt Capital Hockey Academy, now the Hawke’s Bay Hockey Academy, which provides high performance programmes for junior and youth players.

ENDACOTT, Mr Gary Leslie
For services to people with disabilities
Mr Gary Endacott has lived with Cerebral Palsy since birth and has been a Disability Facilitator for the Ministry of Education since 2001 and a mentor and advisor for CCS Disability Action’s mentoring programme since 2009.
In 1995 Mr Endacott was the New Zealand Open Tennis Singles Champion and has achieved first, runner up and third results at the Unofficial World Tennis Championships in Japan. In 1996 he was the first person with Cerebral Palsy to complete the New York City Marathon unaided on foot and in 2001 he was the first person with a significant disability to climb Mount Kilimanjaro. In 2009 he travelled on a voluntary basis to Tokyo to assist the Japanese government with training and implementing policies on managing disability. He has given numerous presentations for the disabled or the wider community in New Zealand, Australia, the Netherlands, the United States, Japan and the United Kingdom. He has worked on supporting disabled students with the Wayne Francis Charitable Trust Transition Project and the SPARC No Exceptions Disability Advisory Group. He worked for eight years with the Child Cancer Foundation, providing one on one support, raising public awareness and funds. Mr Endacott was a Judge for the 2014 Attitude Awards.

FORGESSON, Ms Krystal Dianna
For services to hockey
Ms Krystal Forgesson competed for the New Zealand women’s national field hockey team from 2005 until her retirement in 2014.
Ms Forgesson has been a member of the teams that competed at the Commonwealth Games in 2006, 2010 and 2014, winning silver at Delhi in 2010 and bronze in Glasgow in 2014. She has also been a member of teams that competed at the 2008 and 2012 Olympics and the 2010 and 2014 Women’s Hockey World Cup in Argentina and the Netherlands respectively. She was a member of the team that won bronze at the 2011 Hockey Champions Trophy in Amstelveen and the gold medal winning team at the 2009 Champions Challenge in Cape Town. Throughout her playing career she scored 77 goals making her equal for the record as the current all-time highest goal scorer in the Black Sticks. Her 220 international test caps made her the fourth highest capped Black Stick at the time of her retirement in 2014. Ms Forgesson has played for Auckland in the Ford National Hockey League and was named New Zealand Universities' Māori Sportsperson of the Year in 2007.

GROOME, Mr Antony John (Tony)
For services to Search and Rescue
Mr Tony Groome has been involved in Land Search and Rescue (SAR) since the 1980s.
In the late 1990s Mr Groome joined the Marlborough Coastguard as both a crew and committee member and was later elected as President. During his tenure he developed apparatus to help track emergency beacon signals from distressed vessels. In 2000 he relocated to the Manawatu and has since been involved with Land and Marine SAR in that region. He assisted with trials for the Royal New Zealand Air Force’s NH90 helicopters for use in SAR service. He joined the volunteer ranks of the New Zealand Fire Service in 1989, beginning in Havelock, then Picton and Blenheim, and now Feilding where he continues to serve as a Station Officer, also holding the rank of Deputy Principal Rural Fire Officer. He served for 10 years with the Palmerston North-based Philips Search and Rescue helicopter and took part in numerous rescues in that time. From 2007 to 2009 he was part of a New Zealand Red Cross International response team on exercise in the Solomon Islands, when in February 2009 major flooding saw him establish radio networks on neighbouring islands in support of the recovery effort. Mr Groome has assisted the Police with the planning of several large search operations in support of major criminal investigations, some of which continue in 2015.

GUY, Mrs Kathleen Janet
For services to the tourism and hospitality industries
Mrs Kathleen Guy has been involved with the tourism and hospitality industries for more than 30 years.
Mrs Guy is Group General Manager of Wairakei Resort Taupo and has been Group Manager of Chateau Tongariro Hotel since 1999. She oversaw a 40 room extension to Chateau Tongariro in 2005. She was a Board member of Tourism New Zealand from 2003 to 2008. She was a Board member and Chairperson in 2013 of Destination Great Lake Taupo. She was a Board member, Trustee and Chairperson of Visit Ruapehu in the period 2009 to 2015. She has been New Zealand Hotel Council Regional Chair, and Board member of the Pipiriki House Trust and the Te Kahui Te Pua regional joint marketing initiative. Since 2014 she has been a Board member of Lake Taupo Airport Authority. She is involved with conservation and tourist product development through Project Tongariro and Greening Taupo, including many conservation projects within the Tongariro National Park and tussock planting in and around Taupo. Through Wairakei Resort sponsorship she has been involved with a number of community organisations such as Lake Taupo Hospice and Taupo schools. Mrs Guy was awarded the Hospitality Association of New Zealand Personality of the Year in 2006.

HAWKE, Mr Colin John
For services to rugby
Mr Colin Hawke has given more than 40 years to rugby refereeing at grass-roots and international levels.
Mr Hawke began his refereeing career in South Canterbury in 1972, and made his international debut in 1990. His consistent high ranking culminated in him being appointed one of the world’s first professional referees. He was appointed as a World Cup Referee in 1995 and 1999. From 2005 to 2012, he was New Zealand Referees High Performance Coach, which involved selecting, managing, coaching and mentoring New Zealand’s professional and semi-professional referee panel. In 2012, he became the part-time Referee Education Officer for South Canterbury and North Otago and consistently exceeds the hours and expectations of his role. He is widely respected in the rugby world, and has made a significant contribution as an ambassador for New Zealand rugby. He is respected for his professionalism and passion for refereeing and referee development. Mr Hawke was inducted into the South Canterbury Hall of Fame.

HAYS, Mr Peter Lawrence
For services to accounting and the community
Mr Peter Hays has provided governance, management and reporting support to companies and charities.
Mr Hays has been a member of the Auckland War Memorial Trust Board for the past seven years, and is currently its Deputy Chair. His involvements have included reviewing the content and wording of the Auckland War Memorial Museum Act 1996 to ensure its appropriate amendment on the formation of the Auckland Council; updating the Museum’s governance policies; and overseeing the Trust Deed for the development of the Auckland Museum Foundation. Currently he is Chair of the Museum’s Investment Committee and Deputy Chair of its Audit and Risk Committee. He is also a former President of the University of Auckland Alumni Association and has served as a Trustee of the University of Auckland Foundation for the past twelve years. He is a Past President and Life Member of the Institute of Chartered Accountants in New Zealand. For six years he represented New Zealand on Committees and Council of the International Federation of Accountants. This included involvement in the development of a ‘Professional Code of Conduct for the Accounting Profession’, which was subsequently adopted nationally and internationally. Mr Hays was also an inaugural member of the Accounting Standards Review Board on which he served for nine years.

HEMMINGSEN, Mr Colin Neil
For services to music
Mr Colin Hemmingsen has contributed to music for more than 50 years.
Mr Hemmingsen was the Principal Bassoonist in the New Zealand Symphony Orchestra from the 1970s until 1992 and a member of the New Zealand Chamber Orchestra and the New Zealand Woodwind Quintet. He taught and performed in the United States with the Boston Philharmonia Orchestra and with opera and ballet companies, and was a resident at the Virginia Commonwealth University. He established New Zealand’s first Jazz School for young people and, in 1983, the New Zealand Jazz Foundation where he still teaches and was a Trustee. He also founded the Creative Music School teaching sight reading, improvisation and music theory and has worked as teacher with the New Zealand Youth Orchestra and New Zealand Jazz Youth Orchestra. He was appointed Cultural Ambassador for New Zealand at the Singapore Jazz Festival in 1986. As a Senior Lecturer for 21 years and now part-time tutor at the New Zealand School of Music, Mr Hemmingsen has researched the bassoon in jazz performance, recorded acclaimed performances, including his own compositions, and continues to be in demand as a soloist and accompanist to leading groups and artists in New Zealand.

HUNT, Mrs Sally Mitchell
For services to philanthropy and the arts
Mrs Sally Hunt has supported the Nelson region, particularly through philanthropic support of the arts.
Mrs Hunt and her late husband Robert established the luxury Lodge at Paratiho Farm in the Upper Moutere near Nelson in 1999. Through the Lodge they have supported local artists, purchasing works for display and facilitating private art tours of the Nelson/Tasman region for their guests. The Lodge was featured in numerous tourism promotions for New Zealand. They made a significant donation to the establishment of the World of Wearable Art Museum in Nelson. She has made ongoing contributions to the Bishop Suter Art Gallery, kickstarting a $12 million redevelopment in 2012 with a significant personal donation for the foyer to be renamed in remembrance of late Nelson artist Jane Evans. This followed years of previous donations including for the establishment of an education programme at the Suter which now caters for more than 6,000 students annually. Mrs Hunt has also supported Nelson’s Adam Chamber Music Festival, the Arthouse Installation Project, Salisbury School, the Nelson Art Guide Book and the outdoor winter light festival ‘Light Nelson’, which attracted 45,000 visitors in 2014.

KINGI, Mr Henare Whitu
For services to Māori and broadcasting
Mr Henare Kingi has been involved with Māori radio broadcasting for 25 years and was the Kaumatua and main radio broadcaster for Te Upoko o te Ika, the first total Māori language radio station in New Zealand.
Mr Kingi became the foremost trainer and mentor of dozens of young Māori broadcasters. He has been involved with Te Whakaruruhau o Nga Reo Irirangi, the National Federation of iwi radio stations. He presented for 15 years a Sunday Morning Māori Language service on a voluntary basis. Although retired in 2012, he continues to assist with interviews and karakia for local and national iwi radio by request. He has been a cultural advisor to a number of government offices including the National Library of New Zealand, National Archives and the State Services Commission. He has been Kaumatua to the New Zealand Māori Language Commission. He was one of the founders and committee members of Koraunui Marae, working alongside other Kaumatua to build an urban marae in Stokes Valley for all iwi and cultures. Mr Kingi extended his services to become Chairman for Koraunui Kohanga Reo.

KIRK, Mr Jonathon Charles Wilfred
For services to agriculture
Mr Jonathan Kirk devised a new and innovative spray irrigation system called the K-Line Spray Irrigation System in 1998, a flexible pipe with a series of tough plastic pods housing sprinkler nozzles that cannot be damaged when towed behind a vehicle.
The K-Line is a low cost option ideal for farms that are not suitable for conventional centre pivot or travelling irrigators such as unusual shaped paddocks, steeper terrain or lifestyle blocks. It also disperses water without pooling or runoff. The system is now operating on thousands of hectares in New Zealand and internationally in the United States, Canada, Australia, South Africa, Chile and parts of South America. In 2002 Mr Kirk was approached to devise a plan on getting water from the existing Morven Glenavy Ikawai Irrigation Company water to supply to irrigate some rolling hill country in the Waihao Downs district. He developed several plans on a pro bono basis before a scheme was approved, with the Waihao Downs Irrigation Scheme beginning construction in 2015 to irrigate 23 farms, which will have a great impacting on the district opening opportunities for a larger range of crops and increased production. Mr Kirk has been a member of the Waihao Downs Farmers Committee since inception.

LANGE, Mr Peter Reid
For services to ceramic arts
Mr Peter Lange has contributed to ceramic arts for 40 years.
In the mid-1970s Mr Lange joined Auckland Studio Potters where he has been involved with teaching more than 100 students a week, training several generations of potters, building and firing kilns, organising seminars and giving workshops and helping deliver part of the Otago Polytechnic Distance Diploma in Ceramic Arts. He has held executive positions with Auckland Studio Potters including President and is the current Secretary/Treasurer. In the early 1980s he began creating super-realistic slipcast ceramic sculptures, and his traditional pots of the 1990s were fired with wood and salt in kilns he designed and built himself. He built ‘Performance Kilns’ made of materials such as telephone books, cars, wood and ice and shaped like Rangitoto and guitars, often conducting these performances overseas. He has completed several major works now located in national and international collections and public spaces including The Beehive and Auckland Museum. He has participated in many group shows and has been invited as guest demonstrator in Australia, china, Japan, Turkey, Wales, Dubai and Taiwan. Mr Lange produced a publication recording the first 50 years of the Auckland Studio Potters’ Society.

LIVINGSTONE, Mr Timothy Grant (Tim)
For services to business and the community
Mr Tim Livingstone has been a partner in the Chartered Accountancy practice of UHY Haines Norton (Auckland) Ltd for 30 years, and has been instrumental in developing the firm into a six partner practice with more than 40 staff.
Mr Livingstone is President of the West Auckland Business Club, an initiative he was instrumental in the development of 20 years ago, and which hosts networking functions annually, from which profits are donated to the West Auckland community. He is a member and Fellow of the New Zealand Institute of Chartered Accountants, having served as Councillor and as Chair of the National Public Practice Committee. He has chaired audit, risk management and remuneration committees for the Boards of organisations such as the West Auckland Trusts Service, Languages International Ltd and Learning Media. He has been a Trustee of the Waitakere City Stadium Trust since 2004 and was Deputy Chair of Sport Waitakere. He actively supports the arts and creative sector in West Auckland and is Trustee of the Portage Ceramics Trust and the Lopdell House Development Trust. Mr Livingstone has been a Trustee and Chair of The Dutch Village retirement village.

MACANN, Mr Rodney George
For services to opera and the Baptist Church
Mr Rodney Macann has contributed to opera and to the Baptist Church.
Mr Macann began his operatic career in 1966 in the United Kingdom. In 1969 and 1970 he appeared at the Berlin Festival, Carnegie Hall in New York and Royal Albert Hall in London and returned to New Zealand to debut with the New Zealand Opera Company. From the mid-1970s until the early 1990s he performed with a number of opera companies throughout the world. He sang with many noted conductors and opera singers of the 1980s including Carreras, Domingo and Pavarotti. In 1990 he became Senior Minister of the Wellington Central Baptist Church and from 2002 to 2013 took up responsibility for the Baptist churches of the Wellington Region. He played a key role in the establishment of a primary health care centre for the poor in Te Aro Wellington. From 2006 to 2011 he was National leader of the New Zealand Baptist churches. Mr Macann has continued to perform with major orchestras and opera companies in Australia and New Zealand and more has been a Trustee of the New Zealand Opera School and Kiri Te Kanawa Foundation and a Judge, advisor and Host for Master Classes for the Lexus Song Quest.

MADDIX, Mrs Carole Josephine
For services to netball
Mrs Carole Maddix has been involved with netball for more than 30 years as an administrator, player, coach and umpire and has been a member of the Netball New Zealand (NNZ) Board since 2005.
Mrs Maddix was a member of the Māori and Pacific Advisory Group of NNZ from 2005 to 2011 and helped establish Te Raki Paewhenua Netball. She is an advocate for Māori and Pacific netball, utilising her expertise in all areas of the game to involve more players and officials with the Aotearoa Māori Netball Oranga Healthy Lifestyles national body. She has been an Executive member of Oceania Netball since 2009. She became a founding Board member of Netball North in 1998. In 2001 she began managing the representative teams programme, ensuring sound processes were in place for appointing managers and coaches, selection of teams and performance evaluation. She was appointed Chair of the Netball North Board in 2002. She has also involved in various official positions with Netball North Harbour since the mid-1980s. She was appointed to the Board of Netball Waikato Bay of Plenty in 2014. Mrs Maddix has been a regular volunteer at the local level for Northern Force home games and Netball North Harbour’s secondary and club competitions.

MCBETH, Mr John Stewart
For services as a sports broadcaster
Mr John McBeth is a broadcaster with more than 40 years experience as a radio and television sports commentator.
Mr McBeth was New Zealand’s leading radio rugby commentator for ten years prior to joining Television New Zealand in 1993. He has covered many All Blacks and international tours and Rugby World Cups, and has commentated at eight Olympic Games, seven Commonwealth Games and four Americas Cup regattas as well as numerous other major sporting events. He has worked with the New Zealand Cancer Society as a men’s health ambassador with a particular focus on prostate cancer. In 2014 the Sports Journalists Association presented Mr McBeth with a Lifetime Contribution to Sport Award.

MEALAMU, Mr Keven Filipo
For services to rugby
Mr Keven Mealamu has played as hooker for the All Blacks, the Blues in Super Rugby and Auckland in the National Provincial Championship.
Mr Mealamu made his provincial rugby debut in 1999 for Auckland and debuted for the All Blacks against Wales in 2002. He was a standout performer in the All Blacks 2011 Rugby World Cup victory, turning out in six matches in the Tournament and captaining the side against Japan, and was part of the All Blacks squad which won the 2015 Rugby World Cup. He also played in the 2007 Rugby World Cup. He was test captain of the All Blacks against Scotland in the 2008 Grand Slam tour. In 2012 he became the third All Black to reach 100 test caps. In 2014, he made his 150th appearance for the Blues, becoming the first player in Super Rugby history to make 150 appearances for one team. At the conclusion of the 2015 season Mr Mealamu retired from the Blues and ended his Super Rugby career with 175 caps, 11 of them from a brief stint with the Chiefs in 2002.

MONK, Mr Bernard Hanmer
For services to the community
Mr Bernard Monk has been a central member of the Pike River Families Group Committee since November 2010 when 29 miners were killed in the Pike River Mine explosion.
Mr Monk has been the spokesperson for many of the Pike River families, fronting the group in the media and meeting with government agencies, Pike River Coal Ltd, and Solid Energy. The first meeting of the families was held three weeks after the explosion and came about because Mr Monk believed the group needed organisation and representation to express their views, bringing his lawyer brother-in-law Colin Smith to the meeting to form the Committee. On behalf of the families he has been an advocate for regulatory changes, the families’ contribution to the Pike River Royal Commission, the recovery of the bodies from the mine, and seeking justice for the tragedy. He has contributed thousands of hours in the interests of the families on a pro bono basis. The effective advocacy of the families has been an important factor in improving the health and safety regime in New Zealand. Mr Monk has acted as a counsellor for many grieving family members, putting his own grief to one side to help others come to terms with the loss of their loved ones.

MORRIS, Mr Peter Corran
For services to equestrian sports
Mr Peter Morris is one of the most respected technical officials and judges of equestrian disciplines in New Zealand, having officiated at the highest levels for more than 40 years in a mostly voluntary capacity.
Mr Morris rode competitively from an early age until 21, before transitioning to judging dressage, show-jumping and cross country in his mid-20s. He became an international show-jumping judge in his 40s, the youngest show-jumping judge to gain full international status. He has attended seminars in Vienna and Stuttgart with top international judges from around the world, and shares his knowledge on an annual seminar circuit in New Zealand to help mentor current show-jumping judges. He has been President of the Ground Jury at every major New Zealand show-jumping show, including Horse of the Year. Internationally he has judged in Australia, England, South Africa and Canada, including judging the water jump at the first Queen Elizabeth II Cup at Spruce Meadows in Calgary in 1990. He was Chairman of the New Zealand Equestrian Federation Rules Committee and Chairman of the Equestrian Sports New Zealand Show-jumping Committee from 1992 to 1996. Mr Morris has contributed his time and knowledge at the local community level, judging high school events and gymkhanas in his hometown of Masterton.

MORRISON, Mr Alexander Ross (Ross)
For services to tennis
Mr Ross Morrison and his wife Mrs Dawn Morrison led the development of the world-class indoor/outdoor Albany Tennis Park, the largest tennis facility in New Zealand, with indoor courts constructed in 2000 and 18 outdoor courts finished by 2005.
Mr and Mrs Morrison have overseen the transition from local tennis clubs and the floodlighting and converting of the entire Forrest Hill facility from grass courts to decoralt. They have run Davis Cup Tournaments in Parnell and Albany, and produced the book ‘Tennis on the North Shore’, a history of North Shore tennis from the inception of the first club at Devonport to present day. Mr Morrison was President of Tennis North Harbour from 1987 to 2006 and CEO of Tennis Northern from 2003 to 2007. He has been North Harbour Representative on the New Zealand Tennis Council and the New Zealand Advisory Committee. He was Chairperson of the Tennis Charitable Trust from 2000 to 2014 and remains a Trustee. He is the current Treasurer of the Auckland Umpires Association and the New Zealand Umpires Association. He currently referees for Tennis Auckland, Tennis Northern, Tennis New Zealand and Tennis Seniors. Mr Morrison is a longstanding member of the Pascoe Cup Committee and oversees the Junior Caro Bowl.

Mrs Morrison was the team manager of the North Shore ladies team participating in the Qantas Teams Event for several years and was also team manager of a group of 20 young players competing in the Queensland Shell Junior Masters Tournament in Brisbane in 1992.

MOSS, Ms Jan
For services to the care of disabled people
Ms Jan Moss has provided support and leadership to families of severely disabled people for more than 25 years.
Ms Moss is the mother of a disabled woman and has used her experience and knowledge to support other families. In the 1990s she co-founded and ran for 12 years a national group to provide support and information for families of children with severe epilepsy syndromes. She was instrumental in setting up the Complex Care Group, which provides information and support to families caring for children/young people with complex needs. She now undertakes national leadership roles, including a past role as Chair of Carers’ New Zealand, the national body supporting family, whanau and aiga carers. Through Carers’ New Zealand she played a key role in the development of the government’s first carer strategy in 2008. Ms Jan Moss is recognised for raising awareness and understanding of the needs of families caring for severely disabled people and for her work as a constructive and effective advocate for family carers and their disabled family members.

NELSON, Mrs Linda Patricia (Lindy)
For services to agriculture and women
Mrs Lindy Nelson is the founder and Executive Director of the Agri Womens Development Trust (AWDT), which develops the leadership, business and governance competencies of women in New Zealand agriculture.
The AWDT provides training and support to individual women from ‘on farm’ to community and corporate levels. Mrs Nelson launched the AWDT in 2010 after three years of self-funded research and spent up to 80 hours a week for the following two years as an unpaid executive creating the programmes, culture and bringing strategic partners on board. The Trust now delivers development to more than 500 women per year and the impact of graduates is transforming the sector. Many graduates have gone on to some of New Zealand’s largest agricultural boards and industry organisations, have launched new businesses and led economic and educational development. In 2012 Mrs Nelson was named in Primary Magazine’s top ten women in agriculture, was the 2013 NEXT Business Woman of the Year and in 2014 was one of three finalists in the agricultural business person of the year. As a director of the Wairarapa District Health Board Mrs Nelson has helped develop the sustainability of regional healthcare through the implementation of a strategy for the greenfield development of a new hospital.

NONU, Mr Ma'a
For services to rugby
Mr Ma’a Nonu made his Test debut for the All Blacks in 2003 and was a member of the All Blacks squads that achieved victory at both the 2011 and 2015 Rugby World Cups.
Mr Nonu played his 100th Test against Tonga in the 2015 Tournament, becoming just the sixth All Black Test centurion, and finished his All Blacks career as the most capped All Blacks number 12. He made his debut for Wellington in the National Provincial Competition in 2002, captaining the team in 2005. He made his Super Rugby debut for the Hurricanes in 2003, and also played for the Blues and Highlanders, and played his 150th Super Rugby match in April 2015, just the third New Zealand player to reach the milestone. Mr Nonu also played for the New Zealand Sevens team in 2004 and the Ricoh Black Rams in Japan in 2011 and 2012 and began playing rugby for Toulon in France in late 2015.

PAENGA, Ms Te Aroha, JP
For services to Māori
Ms Te Aroha Paenga has spent her lifetime supporting the preservation of and teaching Te Reo Māori.
Ms Paenga has more recently spent time judging Kapa Haka national competitions at all levels, having played a significant role in the first national Kapa Haka competition in Rotorua in 1972. She was a founding member of the Manutaki Kapa Haka group and is a recognised and respected exponent of poi and other performing arts disciplines. She was part of the original group who established the first Kohanga Reo at Hoani Waititi Marae, along with Sir Pita Sharples and Dame Katerina Te Heikoko Mataira. Ms Paenga is currently a Senior Kaiako at Te Kohanga Reo o Hoani Waititi Marae and has been instrumental in maintaining the standards of reo and tikanga amongst the Hoani Waititi community.

PATTERSON, Ms Jane Frances
For services to sports administration
Ms Jane Patterson is an events manager who has been the Event and Race Director of Ironman New Zealand since 1999.
While working for International Management Group (IMG) Ltd, Ms Patterson managed a range of high-profile events including the New Zealand Golf Open in 2002 and 2003. In 2004 she established The Patter and has provided events management services to the 2007 World Netball Championship, 2004 ‘Nike You’re the Run that I Want’ in Auckland, 2005 New Zealand Golf Open, Port of Tauranga Half from 2005 to 2012, Craigs Investment Partners Tinman triathlon from 2009 to 2011, Pukekohe V8 Supercars, 2012 World Roller Figure Championships, and the 2013 BMX World Championships. She was involved with the Auckland Ironman 70.3 event from 2012 to 2014 and has been involved with the ASB Auckland Marathon since 2014. She is the owner and manager of the REAL Women’s Series duathlon, a non-competitive annual event held in nine locations between 2007 and 2012. Her focus is on community-grown athletics events, partnerships with host communities, safety, and raising the profile of New Zealand as a sporting destination. Ms Patterson was a member of the Independent Training Advisory Group to the Rugby World Cup 2011 and has served on an Independent Advisory Group to UNITEC’s Bachelor of Sport qualification.

RANDS, Mr Malcolm Stewart
For services to business, conservation, and philanthropy
Mr Malcolm Rands has been involved in sustainable business practice and has volunteered and worked in the arts, culture and community development fields.
Mr Rands founded Ecostore Company Ltd 20 years ago, from his home in Northland, and the business has grown to an exporter of environmentally friendly and healthy products with an annual turnover of $30 million. He willingly shares his business knowledge and is involved in the Sustainable Business Network and the Sustainable Business Council. A percentage of profits from Ecostore are channelled into a not-for-profit organisation called Fairground Foundation to achieve environmental projects that benefit the wider community. Mr Rands has a reputation for his dedication and taking a leadership role to create a platform for future development that is sustainable, positive, enduring and inclusive. He is a former member of the Whangarei Community Arts Council Board and founded and ran New Zealand’s first international busking festival. He is a member of the Te Wairoa Trust.

ROSE, Mrs Carol Margaret
For services to the community
Mrs Carol Rose is the Secretary of the Pike River Families Group Committee, which was formed following the deaths of 29 miners in the Pike River Mine explosion in November 2010, for the families of the miners to act as a cohesive collective.
Mrs Rose has undertaken the role of Secretary since the Families Group’s formation and has ensured information has been distributed to the 150 plus wider family members, organised meetings and dealt with considerable amounts of correspondence. She has been a key link with those around New Zealand keen to support the families, as well as government officials and media. She and her husband sold their business to free up her time to focus on the Families Group and she has contributed thousands of hours on a pro bono basis to maintaining the Group’s focus and momentum. She has played a pivotal role in helping individual family members come to terms with the emotional challenges faced after the tragedy. Mrs Rose is a member of the Project Oversight committee and a Trustee of the Pike River 29 Legacy Charitable Trust.

RUSH, Mr Bernard James (Bernie)
For services to the New Zealand Fire Service and Urban Search and Rescue
Mr Bernie Rush has been active in the New Zealand Fire Service, in Auckland, Palmerston North, Hutt Valley and Whanganui since 1971.
Mr Rush was appointed the New Zealand Fire Service National Rescue Manager in 2002, and has been serving as the Area Commander for the Fire Service’s Wanganui region since 2012. He has been instrumental in developing New Zealand’s Urban Search and Rescue abilities, and led training and exercises to promote these skills in his roles in Auckland and Palmerston North. He transferred from the Fire Service for two years in 2002 to support the development of national Urban Search and Rescue capabilities at the Ministry of Civil Defence and Emergency Management. He was instrumental in leading New Zealand to achieve the full United Nations International Search and Rescue Advisory Group Heavy Task Force classification in March 2015. Mr Rush is also a member of the Whanganui Rotary Club and past President of the Lower Hutt Rotary Club, and received a Rotary District Governors Citation in 2012 for ‘his vision in founding the New Zealand Urban Search and Rescue team that responded to the 2011 Christchurch earthquake’.

RYALL, Mr Adam Lindsay Gordon (Phil)
For services to the Deaf and as a philanthropist
Mr Phil Ryall is a Trustee of the Cochlear Implant Foundation and a Director of The Hearing House, having played a key role in the financing and establishment of these services in the early 1990s.
Mr Ryall was involved in setting up and financing mobile clinics located throughout Auckland for the treatment of Glue Ear in children. He established the Phil Ryall Bursary to enable applicants from rural New Zealand to attend Kings College. He has been Patron for several terms of the Clevedon Agricultural and Pastoral Association, having been a Committee member and Past President. He was involved with the Clevedon Takanini Rotary as a Committee member between 1953 and 1983 and President until 1985. He has established a number of property syndicates as an anchor stakeholder in which charities have been able to invest with relative low risk and receive reasonable financial returns, including such charities as Riding for the Disabled and The Hearing House. He is a Life Member and current Patron of the Auckland Polo Club and an Honorary member of the New Zealand Polo Association, having been on the Management Committee. He has fundraised for the Clevedon All Saints Anglican Church and personally funded a new roof for the Māori Anglican Church.

SMITH, Mr Colin Neil
For services to the community
Mr Colin Smith is the Chair of the Pike River Families Group Committee, which was formed following the deaths of 29 miners in the Pike River Mine explosion in November 2010, for the families of the miners to act as a cohesive collective.
Mr Smith has contributed thousands of hours as a facilitator, decision maker and unpaid legal counsellor as the families have dealt with grief and other challenges since the event. He helped the families through the decisions not to prosecute any individual for the tragedy, not to recover the bodies from the mine, and the Royal Commission process. He has been the main contact with the Government on matters of interest to the families. He has been a major advocate for the health and safety reforms that have followed from the tragedy. He has contributed to the West Coast community beyond his involvement with the Pike River Families Group Committee. Mr Smith has been Chairman of West Coast Rugby since 2009, Chairman of the Guardians of Paroa Taramakau Coastal Area Trust, Deputy Chairman of Development West Coast, former District Commissioner of the Westland Pony Club and was Chairman of the Paroa School Board of Trustees from 1998 to 2007.

For services to rugby
Mr Conrad Smith made his All Blacks debut in 2004 and was a member of the All Blacks squads that achieved victory at the 2011 and 2015 Rugby World Cups.
Mr Smith was also a member of the All Blacks squads for the 2007 Rugby World Cup. He is considered to be one of the pre-eminent centres in world rugby. He has captained the Hurricanes in Super Rugby since 2012 and plays for Wellington in the National Provincial Championship. He made his Wellington debut in 2003, and was signed by the Hurricanes in 2004. Mr Smith achieved 100 Super Rugby caps with the Hurricanes in 2014.

SMITH, Mr Rex
For services to cricket
Mr Rex Smith has been involved with cricket in Auckland for much of his life, particularly through the Auckland Cricket Association (ACA) and Cornwall Districts Cricket and Sports Association.
Mr Smith has been on the Board of Directors of ACA for 16 years and has been Chair for a combined total of 11 years, a position he currently still holds. He was one of the driving forces behind the introduction and continued advancement of the Club is the Hub programme that gives financial support to cricket clubs in Auckland, enabling them to be run professionally while assisting them to create sustainable structures. The programme has become a model to be emulated for sport administration in New Zealand and he has led the push to progressively increase funding to clubs, which has led to increased opportunities for cricketers and player number numbers. He has been ACA representative on the Eden Park Trust Board for 14 years. He has been a Board member of Cornwall Cricket since 1990, served as Chairman for nine years and was made a Life Member in 1995. Mr Smith has been the selector and coach for the Cornwall Cricket First team for 26 years.

SOLOMON, Mrs Carolyn Gay
For services to education
Mrs Carolyn Solomon is well known in New Zealand and abroad for her efforts in supporting young women to achieve in the fields of technology, engineering, law and science.
Mrs Solomon served as Principal at Tauranga Girls’ College for seven years, during which time she made it her goal to increase the achievement of Māori in particular. As a result, the school was gifted a carving by Tauranga Kaumatua in recognition of the increased achievement of Māori academically and in sport during this time. She served as the Deputy Senior Head of the United World College of South East Asia for seven years, and was then invited by General Prem Tinsulanonda of Thailand to establish and serve as Principal of the Prem Tinsulanonda International School in Thailand. She worked to secure Japanese investment to establish and run the respected Auckland International College from 2003, through which 95 percent of students attend with full scholarships. Mrs Solomon was appointed to lead the UNESCO Intercultural Workshop in Yokohama, Japan, in 2004, and was awarded the prestigious Woolf Fisher Fellowship for excellence in educational teaching.

STEVENS, Mr Lee Osborne
For services to the community
Mr Lee Stevens is the Director and Co-founder Trustee of the Rua and Clarrie Stevens Charitable Trust, which he and his father founded in 1970 as a memorial to Rua Stevens, his mother, after her early death from Leukaemia, and which has supported a number of arts and service organisations.
As the Trust has grown Mr Stevens created a family philosophy that involved building relationships with a core group of recipients, a family of beneficiaries, rather than giving one-off grants. Since the Trust’s inception the grants to its family of beneficiaries, totalling 200 associations, is now in excess of $1.6 million. Mr Stevens has continued to make annual cash gifts of his own to the Trust. The Trust has supported such beneficiaries as the Clarrie Stevens Memorial Law Scholarship at Otago University, the Dunedin Community Law Centre, Launchpad Youth Programme, Matihetihe Ki Tamaki Trust, the Himalayan Trust, Auckland Philharmonia Orchestra, Royal New Zealand Ballet, Tamaki Pathways Trust, New Horizons for Women Trust, and New Zealand Youth Choir scholarships. He was a founding member of Philanthropy New Zealand and was a member of St John’s Rotary Club. Mr Stevens was a founding Trustee in 2010 and is Foundation Treasurer of the Liberty of Being Me Foundation.

TATTERSHAW, Mrs Julia Margaret (Julie), JP
For services to senior citizens and the community
Mrs Julie Tattershaw has been Administrator of Parata Anglican Charitable Trust since 1971, which oversees the operation of Parata Residence for the Elderly and has seen the facility grow to encompass 26 residential rooms, a day care centre, a retirement village of 27 two-bedroom cottages and a meals on wheels service.
Mrs Tattershaw is an Advisor to the Board of Trustees of the Takitimu Charitable Trust for the elderly. She has been involved with the Holy Trinity Church and the Gore Justices of the Peace Association, of which she was President in the late 1980s. She has been a member of the New Zealand Society of Chartered Accountants and is a Fellow of the Institute of Chartered Accountants of England and Wales. She has been an Honorary Auditor for a number of community organisations including Eastern Southland Plunket Society, Gore Refuge, Gore Women’s Club, St Mark’s Church, Riversdale. She was the District Inspector for Otago Southland for the New Zealand Law Society from 1985 to 2005. Mrs Tattershaw has coached and managed boys and men’s hockey teams.

Ms Rachel Taulelei is an award-winning entrepreneur and business woman who has demonstrated a strong commitment to sustainability, and support for the establishment and growth of other people and companies.
As founder in 2006 and CEO of Yellow Brick Road Ltd, Ms Taulelei was described as having led change in the New Zealand fishing industry, developing an innovative, provenance-based supply of responsibly caught fresh premium seafood to hundreds of establishments across the country. She has taken this demonstrable commitment to kaitiakitanga into her current role as CEO of Maori food and beverage company, Kono NZ LP. With a passion for the primary sector, she has created values-based business models focused on the sustainability of the country’s resources. She demonstrates integrity in every aspect of her relationships with others, has a reputation as someone who operates with an optimistic can-do perspective, and as being eminently approachable. Her directorships include Headwaters New Zealand Ltd, craft beer company Yeastie Boys Ltd, Wellington Regional Stadium Trust, The Sir Peter Blake Trust, Westpac External Sustainable Business Panel, Young Enterprise Trust Supporter’s Council, and she chairs the Wellington Culinary Events Trust. Ms Taulelei was formerly New Zealand Trade Commissioner in Los Angeles, and in 2012 received a Sir Peter Blake Leadership award.

TAYLOR, Mr John Stuart
For services to education
Mr John Taylor is currently Senior Advancement Consultant to the University of Auckland, having retired as Director of Alumni Relations and Development after directing the University’s successful ‘Leading the Way’ campaign which raised more than $200 million.
Mr Taylor was previously Headmaster of both King’s College and Rathkeale College. During his time as Headmaster he was a strong supporter of the girls’ programme at King’s College, overseeing a substantial increase in enrolled female students from 45 in 1988 to 125 by his retirement as Headmaster in 2002. Taylor House was established for the daygirls at King’s College in 2006 in recognition of his support. He was the founding Chair of the Education Forum in 1991, and his current educational activities include membership of the Partnership Schools/Kura Hourua Educational and Scholarship Trust, of the John Drake Memorial Scholarship Trust and of the King’s School Board. Mr Taylor is a member of the NEXT Foundation’s Advisory Panel.

TIMUTIMU, Mrs Wharemawhai Mina (Mina)
For services to Māori and midwifery
Mrs Mina Timutimu has worked as a midwife for more than 50 years.
Mrs Timutimu was the first Māori representative to the National Committee of the New Zealand College of Midwives, and was made an honorary member of the College in 2008 in recognition for the contribution she has made to the College, the midwifery profession, and to Māori health. In 2003, she was appointed to the first Midwifery Council of New Zealand, and was reappointed for a second term in 2006. She was instrumental in ensuring that the Turanga Kaupapa standards for cultural safety were approved by the Council and incorporated into the New Zealand College of Midwives’ Handbook for Practice. She also worked to support the establishment of trans-iwi and inter-professional partnerships between midwives and Māori, and has provided guidance, mentoring and support to innumerable families, nurses, midwives and health workers over the span of her career. In partnership with another local midwife, she established Taranaki’s first Māori community maternity service in 1995 called Nga Puna Ora Te Atiawa, which provided strong support networks for babies and young children. Mrs Timutimu was also instrumental in setting up marae-based Kaumatua support programmes for the elderly.

TOEBES, Mr Gerard Justin (Justin)
For services to basketball
Mr Justin Toebes is one of New Zealand’s leading lawyers and has been involved with the Wellington Saints Basketball Team as President and Treasurer since 1992.
Mr Toebes has helped emerging players develop professionally and supported the Saints team by organising funding through grant applications and corporate sponsorship, making up any shortfall with personal contributions. He has been Chairman of the Wellington Basketball Association and supported them financially when they were facing insolvency. He has assisted numerous young players who made New Zealand representative teams on a user pays scheme, paying for their travel to ensure they did not miss out on any opportunities due to their financial circumstances. He has paid international airfares for players who received scholarships in the United States. He was the Vice President of Basketball New Zealand from 2003 to 2006 and has managed a number of projects for the organisation including reviewing and rewriting their constitution, undertaking a review of the National Basketball League and drawing up license agreements and coach and player contracts on a pro bono basis. He was on the International Basketball Federation Oceania Management Committee from 2003 to 2006. Mr Toebes was involved with the think tank that developed the ASB Stadium in Kilbirnie.

TURVER, Mr Christopher (Chris), JP
For services to journalism, local government and the community
Mr Chris Turver has given more than 50 years to journalism, broadcasting, local government and community activities, mainly in the Kapiti region.
Mr Turver was the first New Zealand Press Association war correspondent with New Zealand combat troops in Vietnam, was wounded in action, and saw further operational service in Borneo and as a correspondent on board a New Zealand frigate to draw international attention to French nuclear testing at Mururoa. He spent 17 years with Radio New Zealand, including Political Editor at Parliament, was involved in the development of Morning Report, and became head of News, Current Affairs and Sport. For nine years he was Greater Wellington Regional Councillor for Kapiti, championing road and rail improvements including Transmission Gully and electric rail extension to Waikanae. As Chief Executive of the Royal New Zealand Coastguard Federation he increased the number of marine search and rescue units around New Zealand and raised funds for new rescue equipment. He is President of the Paraparaumu RSA and Chair of the Electra Trust. Mr Turver earned a reputation as a tireless fighter for local and regional issues including health, transport, environmental protection, boating safety, fire safety, crime prevention, and marine conservation.

WARD, Mr Colin
For services to softball
Mr Colin Ward has contributed to softball in New Zealand as a player, coach, volunteer and administrator since the late 1960s.
Mr Ward played with the New Zealand Men’s Team in the late 1960s and early 1970s, notably achieving third place in the International Softball Federation Men’s World Championships in 1972. He has been Head Coach of the New Zealand B Team, the New Zealand Women’s B Team and the New Zealand Junior Women’s Team. He was Assistant Coach of the New Zealand Women’s Team between 1985 and 1990, until becoming Head Coach of the team from 1992 to 1994. He was a Committee member and Executive Officer of the Wellington Softball Association (WSA) between 1986 and 2012 and is a Life Member, having also served as President from 2007 to 2008. During this time he established WSA Junior Winter Training Clinics, ran midweek competitions such as Slow Pitch League and Business House League, formed the New Zealand Mercantile Softball League and coached Wellington Representative teams. He helped develop Hataitai Park as the home of Wellington softball, fundraising for a scoreboard and floodlights in the early 1980s. Between the 1960s and 1990s Mr Ward has held all offices as a member of the Executive of the Broadway Softball Club.

WOODCOCK, Mr Tony Dale
For services to rugby
Mr Tony Woodcock made his debut in 2002 and has been a mainstay for the All Blacks front row since 2004, as well as playing for the Blues in Super Rugby since 2003 and for North Harbour in the National Provincial Championship since 2000.
Mr Woodcock scored New Zealand's only try in the 2011 Rugby World Cup Final against France, becoming the first All Black prop to score a try in a Rugby World Cup final. He has also played in the All Blacks squads for the 2007 and 2015 Rugby World Cups. He received his 100th cap for the All Blacks in a test match against Australia on 24 August 2013. He is now the most capped All Black prop of all time, and is the second most capped player in Blues history. Mr Woodcock has 149 Super Rugby caps, including 12 caps from a brief stint with the Highlanders in 2013.

WORSFOLD, Captain Mark Robert
For services to the New Zealand Defence Force
Captain Mark Worsfold served as Captain Fleet Personnel and Training from June 2011 until January 2015, during which time he was responsible for the management and delivery of all the Navy’s training and personnel management.
At the time of Captain Worsfold’s appointment the annual attrition rate was 23 percent and the Navy did not have sufficient trained manpower to meet all of its commitments. Over the next three years he re-wrote the strategic model that drove Navy workforce planning, bringing it into line with the Chief of Defence Force’s vision for the New Zealand Defence Force out to 2035. He addressed short-term training issues, transforming the way in which training was organised and delivered within tight financial constraints. As part of his strategy for getting ships to sea he drove the establishment of a Personnel Working Group with New Zealand’s allies. He identified opportunities that addressed some of the personnel gaps in the New Zealand Navy while providing our personnel with the opportunity to assist Australia, the United States, Canada and the United Kingdom in mutually beneficial exchanges. At the conclusion of Captain Worsfold's appointment the Navy’s annual attrition rate had fallen below eight percent and the Navy is better placed to meet its operational demands.

WRIGHT, Mr Darren Walter
For services to the community
Mr Darren Wright has been the Chair of the Christchurch Community Forum since 2011 and has made a significant contribution in this role to the Canterbury Earthquake Recovery for the Greater Christchurch area.
Mr Wright is a member of the Advisory Board on Transition to Longterm Recovery arrangements. He is a Board Member of Canterbury Communities Earthquake Recovery Network and was a member of Christchurch City Council’s Master Plan Joint Advisory Group. He was the Chair of the Sumner Community Residents Association and has been Governor of the Residential Advisory Service since 2013. He is a founding Trustee of the Committee for Canterbury and has been Deputy Chair and now Trustee of the Sumner School Board of Trustees since 2006. Mr Wright is the Deputy Chief Fire Officer of the Sumner Volunteer Fire Brigade and is a Past President of the Mid Canterbury Fire Brigades Sub Association.

ISHII, Mr Eiichi
For services to New Zealand-Japan relations, golf and tourism
Mr Eiichi Ishii has been associated with the Millbrook Resort development in Arrowtown since its inception in 1987 and in 1990 when Millbrook was facing bankruptcy he accepted the role of guarantor to steer the development going forward.
Mr Ishii’s investment grew to full acquisition and included the development of Millbrook’s 27-hole golf course. The resort is set within 200 hectares in the Wakatipu Basin. The golf course hosts major events within the sport with global coverage. The resort has hosted the Commonwealth Heads of Government Meeting in 1995, members of the British Royal Family and a President of the United States of America. Mr Ishii has been instrumental in seeding and supporting golf tourism in New Zealand, with Millbrook becoming integral alongside The Hills to the success of the New Zealand Golf Open. He was a founding member of the Japan New Zealand Executive Club, a group of Japanese business executives with a range of business interests in investments in New Zealand that support a growing relationship with Japan. He is also involved with the Japan New Zealand Business Council and the Japan New Zealand Partnership Forum. Mr Ishii is Chairman of the Too Corporation in Tokyo, Japan.
